Strickland Family Medicine Center uses Phreesia to allow patients to complete forms online prior to their appointment. You will receive a notification from Phreesia a few days prior to your appointment.
It is strongly preferred that you complete your forms via Phreesia. If you are unable to do so, please download and complete the forms below and bring them with you to your first appointment along with your medical insurance card and photo ID. If you need assistance completing form via Phreesia or on paper, please arrive at our office 30 minutes prior to your scheduled appointment time.
